Welcome College of Arts and Sciences faculty! The Academic Monitoring Form   is a great tool for monitoring student progress and assessing retention outcomes.

By the 3rd or 4th week of class, I ask that you please complete an Academic Monitoring Form  for  freshmen and sophomore students in your classes who: 1) are experiencing academic difficulty,  2) have received a “D” or “F” grade, 3) have not been submitting assignments, or 4) have frequent tardies or absences. You should also include your own recommendations for tutoring and/or conferencing on the form. We are asking that faculty complete the monitoring form at least once, but may do more if necessary.


Once I receive the form, I will contact the student for an advising session and make the necessary referrals. At the end of the semester, I will send you a Referral Feedback Form that will generally highlight the results of your submission. You may also log in at any time to view my comments on the students form.
